Registration Information

How to Register:

1. Go on-line to www.dekalb.k12.ga.us/registration

2. Print and fill out the following forms: Student Registration, Household Registration, and Media Release

3. Bring originals AND provide copies of the following items:

· Certified Copy of your child’s Birth Certificate

· Proof of Residency in the Oak Grove District (One of the following: Current gas, power or water bill, contract or lease on home, legal affidavit of residency)

· Child’s Social Security Card

· Georgia Immunization form #3231

· Georgia Hearing, Dental and Vision Form #3300

· Photo ID of Parent/Guardian

4. Submit all of the requested paperwork to the school office anytime between May 2 and May 31, 2016.
