Pre-Registration for 2015-2016 May 1, 2015 - May 31, 2015

9:00 a.m.-2:00 p.m.

Open House/Meet the Teacher Wednesday, August 5, 2015

8:00 a.m.-12:00 p.m.

How to Pre-Register: 1. Go on-line to www.dekalb.K12.ga.us/registration

2. Print and fill out the following forms: Student Registration, Household Registration, and Media Release

3. Provide copies AND originals of the following items:

* Certified Copy of your child’s Birth Certificate

* Proof of Residency in the Oak Grove District (One of the following: Current gas, power or water bill, contract or lease on home, legal affidavit of residency)

* Child’s Social Security Card

* Georgia Immunization form #3231

* Georgia Hearing, Dental and Vision Form #3300

* Photo ID of Parent/Guardian

4. Submit all of the requested paperwork to the school office by May 31, 2015.
