Parents/guardians must provide complete documentation to apply:

​Child's Original Certified Birth Certificate

Proof that the child will be four years of age on or before September 1, 2015

Parent's Photo Identification

Your Child's  Social Security Number

Proof of Residency

Current Month's Gas, Water, or Electric Bill and/or a copy of Lease Agreement

A Residence Affidavit must be provided if current bills are in someone else's name.
